What Clarithromycin tablets are and what they are used for

Clarithromycin belongs to a group of medicine called macrolide antibiotics. Antibiotics stop the growth of bacteria (bugs) which cause infections. Clarithromycin tablets are used to treat following infections:

  • Chest infections such as bronchitis and pneumonia,
  • Throat and sinus infections,
  • Skin and soft tissue infections,
  • Helicobacterpylori infections associated with duodenal ulcers.

Clarithromycin tablets are indicated in adults and children 12 years and older.

Before you take Clarithromycin tablets

Do not take Clarithromycin Tablets if:

  • you are allergic to clarithromycin or other macrolide antibiotics such as erythromycin or azithromycin, or any of the other ingredients of Clarithromycin Tablets.
  • you are taking any of the following medicines:
    • Terfenadine or astemizole tablets (used for hay fever or allergies) – Pimozide (used for psychiatric disorders) – Ergotamine or dihydroergotamine (used for migraine) – Cisapride (used for treatment of digestive problems) Consult your doctor for advice on alternative medicines while taking clarithromycin tablets in combination with these drugs as it can sometimes cause serious disturbances in heart rhythm.

Clarithromycin tablets are not suitable for use in children under 12 years of age.

Take special care with Clarithromycin Tablets if:

  • you are pregnant or breast-feeding
  • you have, or are prone to, fungal infections (e.g. thrush)
  • you are suffering from liver or kidney problems

If any of these apply to you, consult your doctor before taking Clarithromycin tablets. Taking other medicines: Please inform your doctor or pharmacist if you are taking or have recently taken any other medicines, including those obtained without a prescription.   • Theophylline (used to treat breathing problems)

Clarithromycin does not interact with oral contraceptives.

Taking Clarithromycin Tablets with food and drink

Clarithromycin Tablets may be taken with or without food.

Pregnancy and breast-feeding

Clarithromycin Tablets does not usually affect your ability to drive. However, if you feel lightheaded or dizzy, do not drive or operate machinery.

How to take Clarithromycin tablets

Do not give these tablets to children under 12 years. Your doctor will prescribe another suitable medicine for your child. The dose of Clarithromycin varies with the condition being treated. The dose you should take will be on the pharmacist’s label. Always take the tablets exactly as your doctor has told you to. You should check with your doctor or pharmacist if you are not sure. Clarithromycin Tablets should be swallowed with at least half a glass of water.

  • For treating chest infections, throat  or  sinus infections  and skin  and soft tissue infections:
    • Usual dose of clarithromycin tablets for adults and children over 12 years is 250mg twice daily for 6 to 14 days, e.g. one 250mg tablet in the morning and one in the early evening. Your doctor may increase the dose to 500mg twice daily in severe infections.
  • For treating Helicobacterpylori infections associated with duodenal ulcers:
    • There are a number of effective treatment combinations available to treat Helicobacter pylori in which clarithromycin tablets are taken together with one or two other drugs. These combinations include the following and are usually taken for 6 to 14 days:
      • a)  One Clarithromycin 500 mg tablet taken twice a day together with amoxycillin, 1000 mg taken twice a day plus lansoprazole, 30 mg twice a day.
      • b)  One Clarithromycin 500 mg tablet taken twice a day together with metronidazole, 400 mg taken twice a day plus lansoprazole, 30 mg twice a day.
      • c)  One Clarithromycin 500 mg tablet taken twice a day together with amoxycillin, 1000 mg taken twice a day or metronidazole, 400 mg taken twice a day plus omeprazole, 40 mg a day.
      • d)  One Clarithromycin 500 mg tablet taken twice a day together with amoxycillin, 1000 mg taken twice a day plus omeprazole, 20 mg taken once a day.
      • e)  One Clarithromycin 500 mg tablet taken three times a day together with omeprazole 40 mg taken once a day.
    • The treatment combination that you receive may differ slightly from the above. Your doctor will decide which treatment combination is the most suitable for you. If you are unsure which tablets you should be taking or how long you should be taking them for, please consult your doctor for advice.

If you have any kidney problems your dose may be different from that given here, Do not change unless your doctor tells you to do so.

If you take more Clarithromycin Tablets than you should

If you or someone else swallows several of these tablets all together, or you think a child has swallowed any of these tablets, contact your doctor or pharmacist or hospital emergency department immediately. Always take any tablets left over with you, also the box and leaflet as this will allow easier identification of the tablets. An overdose of Clarithromycin tablets is likely to cause vomiting and stomach pains.

If you forget to take Clarithromycin tablets

If you forget to take a dose, take it as soon as you remember. If it is almost time for your next dose, do not take the missed dose and just carry on as before. Do not take a double dose to make up for a forgotten dose.

Do not stop taking Clarithromycin Tablets before you have finished the course:

You must take your medicine for as long as your doctor has told you to. This will usually be for 6 to 14 days depending on the type and severity of infection. It is important not to stop taking the medicine just because you feel better as the infection could return.

Possible side effects of Clarithromycin

Like all medicines, Clarithromycin Tablets can cause side effects, although not everybody gets them.

If any of the following happen, stop taking Clarithromycin Tablets and tell your doctor immediately or go to the casualty department at your nearest hospital:

  • Skin rashes or other allergic reactions have been reported. Allergic reactions may range in severity from mild itchy skin eruptions to severe skin reactions such as flaking and blistering of the skin, peeling of the skin, red pimple like rash and raw inflamed skin.
  • In very rare cases, difficulty in breathing, fainting and swelling of the face and throat can occur. If these occur, seek medical advice immediately.
  • These are very serious side effects, If you have them you may have had a serious allergic reaction or other type of reaction to medicines, You may need urgent medical attention or hospitalization.
  • If you develop severe or prolonged diarrhoea, which may have blood or mucus in it, during or after taking Clarithromycin tablets, consult your doctor immediately.

Other side effects of Clarithromycin tablets may include;

  • a rare allergic reaction called Stevens-Johnson syndrome/toxic epidermal necrolysis (which causes severe illness with ulceration of the mouth, lips and skin)
  • stomach problems such as feeling sick, vomiting, stomach pain, indigestion, or diarrhoea
  • numbness or a feeling of ‘pins and needles’
  • headache
  • joint pain
  • muscle pain or loss of muscle tissue
  • inflammation of the mouth or tongue, discolouration of the tongue or oral ‘thrush’
  • changes in sense of taste or smell
  • tooth discolouration
  • dizziness
  • difficulty sleeping or nightmares
  • anxiety, confusion, loss of bearings, hallucinations (seeing things), change in sense of reality or panicking
  • ringing in the ears or hearing loss
  • liver problems such as hepatitis or cholestasis which may cause yellowing of the  skin (jaundice), pale stools or dark urine.

Rare side effects include;

  • fainting due to low blood sugar levels
  • kidney problems such as raised levels of protein normally excreted by the kidneys or kidney failure
  • inflammation of the pancreas
  • convulsions
  • reduction in the level of white blood cells in the blood (which can make infections more likely)
  • reduction in the levels of blood platelets in the blood (increased risk of bruising or bleeding).

Very rare side effects may include;

  • inflammation of the eye
  • changes in heart rhythm.

If any of the side effects gets serious, or if you notice any side effects not listed in this leaflet, please tell your doctor or pharmacist.

How to store Clarithromycin tablets

Keep out of the reach and sight of children. Do not store above 25°C. Keep in a dry place in the original package. Do not use Clarithromycin Tablets after the expiry date stated on the carton or label. Medicines should not be disposed of via waste water or household waste. Ask your pharmacist how to dispose of medicines no longer required. These measures will help to protect the environment.

Clarithromycin Tablets contain either 250 or 500 mg of the active ingredient clarithromycin.

The tablets also contain inactive ingredients microcrystalline cellulose, croscarmellose sodium, povidone, magnesium stearate, talc, colloidal anhydrous silica, stearic acid and the coating material Opadry 20H 52875 containing hypromellose, hydroxypropylcellulose, propylene glycol, vanillin, titanium dioxide, talc and quinoline yellow lake (El04).

Clarithromycin 250mg tablets are light yellow coloured, oval shaped, biconvex, film coated tablets, debossed with ‘Cl’ on one side.

Clarithromycin 500mg tablets are light yellow coloured, oval shaped, biconvex, film coated tablets, with ‘C and ‘2’ debossed on either side of breakline on one side. Clarithromycin 250mg tablets are available in blister strips of 1, 2, 10, 12, 14, 15, 20, 42, 50, 56 or 100 tablets and Clarithromycin 500mg tablets are available in blister strips of 1, 10, 14, 20, 21, 30, 42, 50 or 100 tablets (Not all pack sizes may be marketed).
